Youth Referral Form

This form is for the use of area social workers, school counselors, youth pastors and others who work with youth who may qualify for & benefit from Kinship's mentoring services.  We want you to be aware of Kinship, and encourage you to suggest it as an option for families.  You should visit with a parent prior to referring the youth; getting their permission to share this information with Kinship.  

Once we receive your referral, we will contact the parent and take it from there.  You will be kept in the loop about the mentoring journey the child is beginning.  Keep in mind the following requirements for youth to join Kinship

1. Child is age 6-18
2. Child lives in a home w/o both parents
3. Child resides in Morrison County or in greater Long Prairie
4. Child would benefit from a friendship-based relationship with a volunteer mentor
5. Child does not have extreme needs or behavior which are beyond the scope of a mentoring program and are better address by therapy / counseling povided by professionals)
6. The child's family is in support of having a mentor through Kinship.

If the child fits these requirements, please complete and submit the form below.  The information is entered & submitted through Kinship's secure website.   At that point, you can work with the parent to complete the youth application.
